Office of the Registrar

The Office of the Registrar delivers a wide variety of services to the academic community. The office aims to provide services in an well-organized manner that allows the members of the academic community to use the services with minimum effort and extreme contentment. The Registrar’s office is in charge for student admissions and registration, grade reporting and transcript of records generation, and maintenance of academic records.

Registrar is one of the important front liner of the university, serving our clients--the students and is responsible for the accuracy, integrity, and security of academic  records. Our standing policy is always to respond  to authentic requests for information or services as quickly and precisely as possible.

 

FUNCTIONS:

The Director for Student Records and Admission is responsible for the supervision and management of all administrative and operational functions of the office of the Registrar which is considered as the Central Office of Student Records and Admission.

The Director shall, in accordance with the vision and objectives of the Office of th Student Records and Admission, ensure the integrity, accuracy, and security of all academic records and current and former students of the University. In particular, he/she shall perform the following functions;

  1. Assist in the development and maintenance of sound student records –keeping and management in the division.
  2. Help meet enrollment goals for the assigned target per curricula offering of the University;
  3. Provide accurate, certified, validated and authenticated information and documentation of the students’ credentials in accordance with the existing University’s policy and in total realization of the University’s vision, mission and objectives;
  4. Facilitate effective students’ registration and enrollment;
  5. Process the articulation of students’ transfer credits, graduation and certification of baccalaureate, associate, graduate and post graduate degrees, enrollment and degree verification, production of official transcripts, diplomas, and commencement ceremonies;
  6. Ensure and sustain proper creation, use and disposal of records observing sound records keeping, management and policy of all the students’ academic records, so as to achieve efficient, transparent and accountable governance;
  7. Coordinate with the Campus Directors/ Deans of the different colleges in curricular offering and other related activities of the campus/ colleges;
  8. Coordinate with Camps Directors/Deans of the different Campuses in providing the University, Board of Regents, CHED, PRC, CSC and other agencies with the updated and accurate reports on enrollment, graduates and other data/information requested from the University;
  9. Decide on academic- related matters involving student credentials, evaluation of subjects and/or credits earned, academic placement and eligibility for graduation.;
  10. Recommend policies for presentation and approval by the appropriate bodies;
  11. Implement approved policies and admission decisions;
  12. Assist in the development, maintenance and execution, of the division’s communication plan to prospective and admitted students;
  13. Plan and implement assiduous service to the University and its greater community by responding in a professional, efficient, courteous, and sincere manner towards all its clients;
  14. Develop and realize appropriate enhancement program in updating and upgrading the professional skills, competencies, and knowledge of the office personnel in the division through relevant in-service trainings and/or scholarship, etc.;
  15. Oversee the collation of all the University terminal reports on student records, including related data from other campuses as deemed necessary for submission and/or for publication.
  16. Upgrade the office system according to the needs and change of time;
  17. Validate ID cards and collect grades from the faculty; and,
  18. Perform other functions as maybe assigned by the University President, the Vice President for Academic Affairs as well as the other offices.

REQUEST FOR TRANSCRIPT OF RECORDS (TOR):

Present your campus clearance before you are given a request form.  (No clearance, no request form).

  1. Fill out the request form and ask the registrar, clerk to indicate the payment for your request and proceed to the accounting to review if you have back accounts or unliquidated amount.
  2. Then proceed to the cashier for payment.
  3. After paying the amount, proceed to the registrar and file your request. Wait for your claim slip.
  4. Claim your requested TOR on the date indicated in your claim slip.
  5. The requested TOR will be release at maximum of 10 working days.

REQUEST OF DIPLOMA:

  1. Pay for documentary stamp at the cashier’s office.
  2. Return to the Registrar and claim your requested Diploma.

REQUEST FOR CERTIFICATION, AUTHENTICATION AND VERIFICATION (CAV):

  1. Ask for a request form with indicated payment.
  2. Pay the indicated amount at the cashiers’ office.
  3. Return request slip with OR to the registrars’ office,
  4. Wait for your claim slip
  5. Claim your requested CAV on the date specified in your claim slip.
  6. Requested CAV will be released at a maximum of 5 working days.

 

REQUEST FOR CERTIFICATION FOR MASTER AND DOCTORATE DEGREE;

  1. Ask for a request form with indicated payment from the registrar clerk.
  2. Pay to the Cashier’s office..
  3. Return request form with OR to the registrar’s office.
  4. Wait for your claim slip.
  5. Requested certification will be release 5 working days.

 

CERTIFICATION OF GRADE AND ENROLLMENT (Undergraduate Students)

  1. Request will be released within the DAY.
