Identified Direct and Secondary Contacts are required to submit a Certificate of Quarantine Completion issued by your respective BHERT upon return to office. In the event of non-issuance of the said certificate for the Secondary Contacts, you are to fill out a Health Declaration Form at the SKSU Health Services Division. Everyone is mandated to strictly follow health protocols at all times.
