The SKSU Graduate School would like to inform you that the online pre-listing/application for incoming freshmen for First Semester A.Y. 2021-2022 is set on JULY 1 to JULY 10, 2021.
Below are the three (3) easy steps on online pre-listing/application:
Step 1. Download the Personal Information Sheet (PIS);

Step 2. The filled-out PIS together with pre-listing requirements should be scanned and saved as PDF in a single file with a file name of your last name, given name, and the program would like to enroll (Ex. Dela Cruz, Juan MAED-EM); and
Step 3. Upload your scanned documents through this link: https://forms.gle/stHPWT9CsyCVmGs59.
Pre-listing/application REQUIREMENTS:
- Transcript of Records from the last school attended
- PSA-Authenticated Birth Certificate
Notes:
- Send your application not later than the last scheduled date.
- Avoid multiple submissions of your requirements, or your application will be denied.
- Please be informed also that virtual interview shall be scheduled after the pre-listing to be posted on our University Website (sksu.edu.ph), likewise, be updated with our latest announcement.

- Honorable Dismissal from last school attended.
- PSA-Authenticated Birth Certificate.
- Permit to study
- 2×2 picture (2 copies)
- 1 valid ID
- 2 brown long envelope
